    Opportunity Information
    Sutter SeniorCare (SSC) is seeking a geriatrician staff physician with fellowship training in Geriatrics or Hospice/Palliative Care or significant experience. You will be responsible for the provision of primary care to SSC participants. This includes evaluating and treating participants, managing their care, coordinating referral and follow-up with specialists, health education and disease prevention. Also, as Medical Director, this position is responsible for overseeing quality assessment, improvement and utilization review, clinical supervision of SSC clinicians, and will work with Sutter clinical leaders to integrate PACE into system population health strategies.

    SSC?s model of care is known as the ?Program of All-Inclusive Care for the Elderly? or PACE.
    PACE is a Medicare and Medicaid program that helps people meet their health care needs in the community instead of going to a nursing home or other care facility. The PACE model has been recognized as the most appropriate model of comprehensive care for the frail elderly population. SSC has a proud history as one of the original PACE programs founded in the country and is the only PACE program that serves the Sacramento Region.

    Palo Alto Medical Foundation has a per diem opportunity for a BE/BC fellowship-trained geriatrician (IM or FP) to join our growing Geriatric and Palliative Medicine Department in Santa Cruz, California.

    Our newer colleagues will work alongside us to provide post-acute and long term patient care in skilled nursing facilities and provide sub-specialty geriatrics care with a dynamic interdisciplinary team.

    Our group is passionate about supporting patients throughout the continuum of care. In addition, we are all very involved in quality improvement and research projects that have both local and national impact. There are opportunities for program development as well.

    Applicants should have a current California Medical License, DEA Certificate and eligibility to participate in Medical/Medicare. BC/BE in palliative care is a plus.

Santa Cruz County is also an agriculture center, known for its production of strawberries, raspberries, artichokes and Brussels sprouts.
