    Sutter Medical Group is seeking a Physician Assistant or Nurse Practitioner to join an established Internal Medicine practice in Davis, CA. This position is full time with full benefits. Candidate will co-manage acute, continuity and preventative care for a complex panel of IM patients, in a team-management approach with the physicians. Duties may include, but not limited to, seeing new and follow-up patients for basic history and physicals, female wellness exams, prescription refills and In-Box management. Office hours are Monday-Friday, 8:30 - 5:00 p.m. Candidate must also be comfortable with Electronic Medical Records. Two - five years experience in Internal Medicine or Family Practice is preferred. However, all candidates will be considered with the appropriate training, background, and interpersonal skills. Applicant must be Nationally Certified and eligible for a California and DEA License.

    The nurse practitioner is a licensed independent practitioner who is responsible for managing health problems and coordinating health care for the preoperative patient in accordance with state and federal rules and regulations. The NP follows the nursing standards of care regarding assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plan, and evaluation to patient status. Preoperative preparation and clinical management is conducted in collaboration with other health care team members, specifically the Anesthesiologist Consultant and/or Medical Director.


    Essential responsibilities include (but are not limited to) the following:

    • Functions independently to perform in-person or phone evaluations on all scheduled perioperative clinic patients (Complete medical history, Perform physical exam if in-person evaluation, Medicine reconciliation, Complete preop nursing assessment, Review advance directive, Scan outside documentation in EHR)
    • Presents medically complicated patients to anesthesiologist consult
    • Provides appropriate patient and family education
    • Performs lab draws or ekg if tech unavailable
    • Formulates an assessment and plan to prepare the patient for surgery, including ordering and following up on diagnostic and therapeutic tests relative to patient?s medical problems or age specific needs
    • Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities in preparation for surgery
    • Enters preop orders into the EHR
    • Monitors the effectiveness of interventions
    • Collaborates with multidisciplinary team members when needed and makes appropriate referrals
    • Facilitates staff, patient, and family decision-making by providing educational tools

    • Minimum Qualifications:

      1.
      • Graduate of an accredited Nursing school.
        2.
      • Graduate of an accredited Nurse Practitioner program.
        3.
      • Current California License as a Registered Nurse.
        4.
      • Current California License as a Nurse Practitioner
      • Possess Furnishing Number from the State of California, Board of Registered Nursing.
        relative to patient?s medical problems or age specific needs

    Sutter Medical Group is seeking a Physician Assistant or Nurse Practitioner to support Sutter Medical Group in performing Annual Wellness Visits in the senior population. This position is 0.8 - 1.0 FTE with full benefits. Wellness visits will be conducted in Yolo, Sacramento, Elk Grove and Folsom based on needs, so travel will be required. Candidate will specialize in chronic condition chart review, coding and documentation as well as counsel patients with recommendations for wellness including fall risk, depression, diet and activity in a team-management approach with the physicians. Candidate must also be comfortable with Electronic Medical Records. Three or more years experience in Internal Medicine or Family Medicine is preferred.
