    For children 0 to 11: Complete and submit all well-child forms online through My Health Online's child proxy access feature. Sign up for My Health Online. Already a My Health Online member? Log into your account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."

    You can also download, print and complete the age appropriate well-child form at home - then bring it to your visit. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.

    Philosophy of Care

    I consider it a privilege to be entrusted by my patients and their family members. I strive to serve my patients in the manner that I would hope that my own family members would receive from their physicians. My goal is to provide compassionate and thorough care.

    My training and career has taken me from California, to the deep South, to the Midwest and now back to California again. Each step in this journey has taught me about diversity, culture and adapting to various environments. I am delighted to have returned home and look forward to serving the community.
