For children 0 to 11: Submit all well-child forms online through My Health Online's (MHO) child proxy access feature. If you don't have proxy access, log into your MHO account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."
You can also download, print, complete and bring to your next visit the age appropriate well-child form. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.

I always consider it a privilege when families invite me to participate in the care of their child. I hope to provide a child friendly and comfortable environment for my patients. I make it a priority to stay up to date, so that I can provide my families with evidence based knowledge, so we can work together to make the best decisions for your individual child's health. I also encourage open conversation and hope my families feel comfortable contacting me and our medical team whenever they have questions or concerns. And, I encourage families to use our phone and email services. A family-centered, supportive practice focusing on preventative medicine is my goal.
