For children 0 to 11: Complete and submit all well-child forms online through My Health Online's child proxy access feature. Sign up for My Health Online. Already a My Health Online member? Log into your account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."
You can also download, print and complete the age appropriate well-child form at home - then bring it to your visit. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.

My philosophy of care includes providing the best personal service to my patients in an open-minded, timely and approachable manner. I believe that one of the best things a doctor can do is to educate patients about health and chronic diseases and how to make them feel empowered and in control of their health. I feel that medicine is an art. There may not always be one right answer. Often, there can be multiple ways to treat and manage a person's life and illness, to fit according to each patient's personal beliefs and culture.
One of the greatest joys of being a family medicine physician is getting to know not only the patient but his or her family members, as well. I love hearing about their relations and social lives outside of the 'medical clinic' because I get to know the patient as a whole and also see how much their personal lives may affect their medical lives. I strive to be personable and approachable to my patients and try to keep things in a lighthearted manner.
