For children 0 to 11: Submit all well-child forms online through My Health Online's (MHO) child proxy access feature. If you don't have proxy access, log into your MHO account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."
You can also download, print, complete and bring to your next visit the age appropriate well-child form. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.

I love pediatrics and enjoy helping in the growth of the child and family. A family-centered approach and evidence-based medicine are the two pillars of my pediatric practice.
