If you are expecting a baby, let us help you check a pediatrician's availability by submitting a newborn referral form.
For children 0 to 11: Submit all well-child forms online through My Health Online's (MHO) child proxy access feature. If you don't have proxy access, log into your MHO account, click on the "Health Record" link on the left menu, then click "Request Proxy Access."
You can also download, print, complete and bring to your next visit the age appropriate well-child form. Save money, too! Bring in any other forms you need completed for your child and you'll avoid getting charged the usual form fee.

My philosophy of care is rooted in prevention and education -- educating parents and children from the earliest ages how to best raise their children to be healthy people and how to adopt healthy lifestyles. I enjoy supporting both the children and their families through sickness and health. When medical problems do arise, I want to quickly initiate therapy and educate and empower the family to ensure compliance and optimal outcomes.
