    Advanced Illness Management (AIM)®

    Helping You Take Control of Your Health, So You Can Focus More On Your Life

    At Sutter Health, we've seen how the right support can help patients with advanced chronic illnesses live the kind of life they want. When given skilled, respectful coaching and care, patients can better manage their health.

    That's why we created Advanced Illness Management (AIM), a program that partners with your doctors to provide you with the nursing and other support you need to manage your illness and live your life.

    Care is Customized to Meet Your Needs

    Once your AIM team understands your health issues, lifestyle, and personal preferences, they will work with you to tailor a care plan that meets your needs.

    Once you enter the AIM program, we will:

    • Help You Plan for Your Future

    Compassionate Professionals Provide Care & Comfort

    Every AIM staff member is a committed, caring professional who has the experience, knowledge and training to help you manage your complex illness. AIM's Medical Director is available to consult with the AIM team and your own physician about your care.

    Ease Your Transition From Hospital to Home

    Your participation in the AIM program begins with a visit from an AIM nurse or social worker, whether you're at home or in the hospital. Over the first few weeks of your care, AIM staff work to meet your immediate needs, and to develop a thorough understanding of your health history, the treatments and medications you are receiving, and your current condition.

    We will continue to support you and your physicians' goals of care by providing you with over-the-phone support and in-person visits in the home or at your doctor's office as needed. If you have to return to the hospital, AIM staff will be there to support you.

    Addressing the Needs of Your Family

    Your AIM team also provides support for your family, and helps them understand anything about your condition that you want them to know.
