To ensure the best possible outcome of your upcoming surgery, you must complete a two-step pre-registration process at Mills-Peninsula. Everyone who has a test or procedure at Mills-Peninsula must complete this process.
If you are uncomfortable speaking English or are hearing impaired, it is important to tell your doctor’s office to notify us so that an interpreter can be scheduled.
Step 1: Pre-Registration
You will be contacted by the Admitting Department for an appointment with a registered nurse. That appointment will be by phone or in person. If you have not been contacted within five days of your scheduled procedure, please call:
- Mills-Peninsula Admitting Department at (650) 696-5373. After 5 pm, please call (650) 696-5594 for same-day surgeries. Call (650) 696-5754 for pre-admission testing.
- Mills-Peninsula Surgery Center at (650) 696-4417.
Be sure to have your insurance information with you.
Step 2: Health History
A nurse will review your health history with you and explain what tests are necessary based on your health status before your surgery or procedure. For your convenience, we have two convenient laboratory locations: Mills Health Center Lab and Mills-Peninsula Medical Center Lab. You can ask any questions you have at this time.
Before your pre-admission appointment, you will be asked to download, print and fill out the following forms:
- Anesthesia Pre-operative Evaluation Form
- Medication Reconciliation Form — A list of medications that you are currently taking, including all over the counter or herbal supplements.
- Advance Health Care Directive
If your surgery will be performed at Mills-Peninsula Medical Center - Burlingame Campus, fax forms and reports to (650) 696-5753.
If your surgery will be performed at Mills-Peninsula Health Center - San Mateo Campus, fax forms and reports to (650) 696-4373.
If you decide to have any preoperative lab work, EKGs, X-rays or other tests at locations not affiliated with Mills-Peninsula, please fax those reports to the above fax numbers.
Step 3: Pre-admission Appointment
Bring your insurance information and photo identification to the hospital for your pre-admission appointment (if in person), or bring them on the day of your surgery. For maps and facility information, click on the location of your surgery: Mills-Peninsula Surgery Center, Mills-Peninsula Medical Center, Dorothy Schneider Cancer Center or Mills Health Center.
Guardians of a child younger than age 18, or conservators acting as the legal representative of an adult patient who is unable to consent on his or her own behalf, must bring documentation that establishes them as the person authorized to make healthcare decisions for the patient.
