The diabetes educator will do a complete assessment based on information that you provide and that is available in your medical records such as additional diagnoses and labs. After the assessment, the educator will tailor an educational plan based on your individual needs.
Patients are encouraged to attend the next Living Well with Diabetes Class which will review the basics of diabetes management, including:
- Understanding Diabetes and Risk Factors
- Diet/Nutrition
- Blood glucose monitoring
- Exercise
- Medication management and preventing complications.
