Description
This project is designed to provide information about current practices for referring patients diagnosed with advanced (stage IV) cancer to palliative care and more broadly for discussing their goals of care. We will use a combination of interviews with patients and care teams, and evaluation of electronic health record data to identify current practices and areas for improvement.
