As a small pilot, exploratory study, it is conducted as collaboration among the Palo Alto Medical Foundation Research Institute, Sutter Health Research, Development & Dissemination (RD&D), and administrators at Alta Bates Summit Medical Center.
The primary purpose of this study is to collect information that can be used to inform patient care and, when appropriate, identify strategies to more efficiently transition high utilizers to more appropriate care arrangements that can meet their needs more effectively than the inpatient care setting.
Co-Investigator is Alice Pressman, Ph.D., Sutter Health RD&D.
