California Pacific Medical Center (CPMC) has long been on the forefront of advanced heart failure therapies. CPMC was the first private medical center in the western United States to perform a heart transplant at the program’s inception in 1984. Since then, we have performed more than 600 heart transplant operations. CPMC has a strong history of innovation in the field of mechanical circulatory support (MCS) since the beginning of the program. The founding surgical director was integral in the design of early ventricular assist devices (VAD), setting the stage for the development of today’s third generation devices.

CPMC was the first center to support a patient with a biventricular MCS device while awaiting heart transplantation. CPMC has been integral in multiple major clinical trials in MCS technology including the recent MOMENTUM 3 trial of the Heartmate 3 ventricular assist device. Building on this legacy, the program saw new growth and innovation starting in 2016 under the medical and surgical leadership of Michael Pham, M.D. and Brett Sheridan, M.D. The program has grown significantly in volume and has had sustained excellent patient outcomes in both transplantation and mechanical circulatory support.

The one-year ACGME accredited fellowship in Advanced Heart Failure and Transplant Cardiology was founded in 2020. Our program is the first advanced subspecialty fellowship program in CPMC’s long history of cardiology education that began in 1959 with the inception of the fellowship in Cardiovascular Diseases. Our program provides our fellows with comprehensive clinical training, rich academic resources, and a dedication to teaching from its faculty that will allow our trainees to develop strong foundations for practice and become leaders in the field.

Heart Failure and Transplant Cardiology Fellowship at Sutter Health affiliated, state-of-the art California Pacific Medical Center in the heart of San Francisco.","duration":"PT4M57S","transcript":"JARED HERR: Here at CPMC, we build upon a legacy\nthat started many years ago.\nAnd with that, we understand that we\nhave a significant responsibility\nto train the future of our field.\nIt was only natural for us to develop an advanced heart\nfailure and transplant cardiology fellowship program.\nCPMC been training cardiologists since 1959\nwhen the general cardiology fellowship began.\nBut we, in 2020, are the first subspecialty within cardiology\nto develop an independent training program.\nAnd we have a one-year ACGME accredited advanced heart\nfailure and transplant cardiology fellowship program.\nAnd this program is designed to train the future advanced heart\nfailure cardiologists and leaders within our field.\nMICHAEL PHAM: The advanced heart failure and transplant\ncardiology fellowship provides a unique opportunity for fellows\nto be involved in a rapidly expanding program that\nserves a broad geographic region across northern California.\nWe're part of an integrated health care system, which\nconsists of 24 hospitals, as well\nas a massive ambulatory footprint of clinics,\nurgent care clinics, and outpatient surgery\ncenters throughout northern California.\nOur strength is that Sutter is an open network, which\nmeans that we care for advanced heart failure patients that\ncome both within and outside the network.\nWe were the first private hospital\nto perform heart transplants in 1984\nand have performed more than 500 heart transplants since.\nLast year, we performed 22 heart transplants, 33 durable LVADs,\nand over 60 ECMO cannulations.\nJARED HERR: Our patient population here\nprovides ample breadth of management\nand clinical experience in all aspects of heart failure care,\nfrom the stable chronic outpatient\nto the acute cardiogenic shock patient who requires\ntemporary mechanical support.\nMICHAEL PHAM: At CPMC, you have exposure\nto all the subspecialties of cardiology,\nincluding advanced cardiovascular and\ninterventional imaging, structural heart,\nelectrophysiology, and congenital heart disease.\nOur ICUs are staffed by nurse practitioners\nand intensivists 24/7.\nAnd we have hospitalists that write orders and respond\nto nursing questions on the floor, which\nmeans that our Fellows can focus on the big picture\nof patient management and doing procedures\nand not on doing scut.\nJARED HERR: One of the greatest aspects of this program\nis that we are a small program.\nIt allows for fellows to have really great one\non one interaction with faculty that you wouldn't necessarily\nobtain at a larger program.\nThat will allow our fellows to develop\nlong lasting relationships with their faculty\nand develop both clinical and academic skills that\nwill allow them to be leaders in the field of cardiology.\nBRETT SHERIDAN: The trainee, the fellows,\nthey get to bridge the entire expanse\nof the journey of the advanced heart failure patient,\nfrom pre-admission to the hospital,\nthrough their hospital experience,\nadmission of cardiogenic shock patients\nthat are highly dynamic and unstable.\nAnd they're a part of that journey,\nwhether it's from a medical non-interventional perspective,\nor whether it's through a surgical perspective.\nThey can be a part of the donor runs,\nand they will be, actually.\nOr even part of the intraoperative experience,\nif they so choose, either by reviewing\nthe transesophageal echo or viewing the procedure\nor even scrubbing in.\nAs far as our heart transplant outcomes, we are,\nif you look at the scientific registry\nof transplant recipients, we're top 10 in the country.\nRight now, we're ranked number two.\nWe're number one in the state of California\nfor one-year survival, time on wait list prior\nto transplantation, and mortality rate on the wait list\nprior to transplantation.\nSo using those three metrics, SRTR\nhas put us number two in the country.\nAnd so we feel like whatever we're doing,\nwe're doing it right.\nAnd it's a reflection of the esprit de\ncorps and the multidisciplinary team spirit\nthat we apply every day here at CPMC.\nJARED HERR: Our fellows will have ample opportunity\nto participate in original research, leading\nto publication or presentation at national meetings,\nas well as in peer reviewed journals.\nMICHAEL PHAM: At CPMC, we have an existing research\ninfrastructure within our GME program\nto help residents and fellows design a project\nand submit a protocol for IRV review.\nAdditionally, we're building a robust database within REDCap\nto capture all of our patients who\nrequire acute circulatory support devices, heart\ntransplants, or durable LVADs.\nThis will enable us to track the outcomes of our patients\nover time and to answer research questions.\nI'm very excited about this database as a resource\nfor our future fellows.\nJARED HERR: The San Francisco Bay Area\nis an outstanding place to live.\nFrom the rich culture opportunities, great weather,\nand ample outdoor activities, it has something for everybody\nhere in this area, and you would love living here."}
