The Internal Medicine Residency Program at CPMC features a high-level of patient care responsibility in an academic, university-affiliated hospital environment. We are one of the major academic affiliate Internal Medicine training sites for Dartmouth Medical School with strong academic ties with the University of California, San Francisco (UCSF) School of Medicine. Our program graduates are well-equipped for careers in primary care, hospital-based practice or further training in subspecialty medicine. CPMC is one of the largest quaternary referral centers with a large catchment area of Northern California, the Central Coast and surrounding regions.

The doctors, hospitals and residents of CPMC provide a consistent level of exceptional and personalized care to patients. Our hospital earned recognition as among the top hospitals in California. We are high-performing in cancer, gastroenterology, nephrology, orthopedics, congestive heart failure, cardiology, and chronic pulmonary diseases. We are also committed to diversity and inclusivity and were named a leader in LGBT Healthcare Equality. We have also been awarded the AGC CoCare: HELP™ Center of Excellence designation for expert implementation and proven success of our Hospital Elder Life Program (HELP) program. CPMC received this elite Center of Excellence status based on demonstrating the highest level of best practices and is one of only five organizations in the world to receive this level of geriatric care recognition.

## Medicine Ward

We have Medicine teams comprised of one attending, one senior resident, two interns, and up to 2 medical students. The patient cap for each Medicine team is 16 patients (8 patients per intern). Rounds are a combination of team workroom and bedside rounds at the senior residents and attending's discretion.

We have a drip admission system where the medicine teams alternate admissions daily.

We do not schedule any 24-hour call shifts at CPMC.

## Cardiology

We have one Cardiology telemetry team each comprised of two senior residents, one intern, and up to 2 medical students. A Cardiology fellow is always present and helps run rounds and admit patients. The intern’s patient cap is 8 patients and the team cap is 14 patients. Our cardiologists provide small-group didactic teaching Tuesday-Friday from 8:00-9:00am. Rotating topics include Advanced Heart Failure, Electrophysiology, and general inpatient Cardiology.

## ICU

We have one ICU team comprised of two senior residents, two interns, and a medical student. A Pulmonary & Critical Care Medicine Fellow is present to help facilitate rounds and assist residents with procedures. The patient cap for the ICU team is 15. Our intensivists provide didactic teaching on weekdays and topics include high-yield foundations of critical care medicine and frequently include mock codes.

We do not schedule any 24-hour call shifts at CPMC.

## Night Rotations

There are 3 main night rotations:

- The Crosscover intern cares for patients on the teaching services overnight. A senior resident is on service to assist Crosscover interns at the beginning of the year.
- The Night Float Admitting team is comprised of one in-house admitting attending, one senior resident, and one intern. They take admissions for the medicine, cardiology, and oncology services overnight. Interns are capped at a maximum of 5 admissions.
- The Night ICU team is comprised of one in-house ICU attending, one ICU senior resident, one ICU intern resident. This team is responsible for admitting patients in both the cardiac and medical ICU. They also provide overnight coverage for patients in these units. There is always an in-house ICU attending available overnight.

Interns and residents always have two nights off per week while on night rotations.

## Ambulatory Primary Care Curriculum

We offer an outpatient curriculum where categorical residents follow their own patient panel from day 1 of internship. Our clinic is located in the heart of the Pacific Heights neighborhood in San Francisco. It serves a diverse patient population covered through Medicare, Medi-Cal, private insurers, and North East Medical Services (NEMS) (a local non-profit insurance providing insurance to underserved populations of the Bay Area).

The Ambulatory Primary Care rotation provides a good balance between clinical medicine and didactics. Here is an example of a resident's schedule:

| | **MONDAY** | **TUESDAY** | **WEDNESDAY** | **THURSDAY** | **FRIDAY** |

Didactics on Ambulatory Primary Care:

- Tuesday Core Curriculum: 2 hour didactic on a core Ambulatory topic
- Wednesday Ambulatory Morning report: Resident-led presentations coupled with Chief-led discussions related to outpatient clinical cases
- Thursday Communication: A time to reflect and debrief difficult situations in small groups with residents and attendings
- Friday Evidence-Based Medicine: This is an attending-led weekly small group session where we learn how to analyze journal articles, review biostatistics, and critique study design."
- We also have a "Pre-Clinic Talk" daily from 1:00 – 1:30 pm on core ambulatory topics based on the Yale Outpatient Curriculum

heart of San Francisco.\nWe're the quaternary referral center\nfor Sutter Health, one of the largest health\nsystems in the region, serving Northern California, Nevada,\nand Oregon.\nOur residency program is a major internal clerkship\nsite for Dartmouth Medical School\nand hosts a visiting fourth-year medical student program.\nYOHANNA KHACHIYAN: So we are the current chief residents,\nand we're so excited that you've chosen to apply to our program.\nWe've reviewed your applications,\nand we can't wait to get to know you all better.\nYou've each demonstrated a strong sense of purpose,\nyou've shared a compelling story, displayed\nintellectual curiosity and enthusiasm for learning,\nand you've shown a deep commitment to clinical medicine.\nOLIVIA WANG: We're excited to show you\nwhat makes our program special.\nChoosing a residency program is a two-way process.\nSo let's take a closer look at our community and the hospital\nwhere you'll be training in.\nPRIYANKA SHINDGIKAR: Our hospital\nsits at the intersection of Pacific Heights, Japantown,\nChinatown, and the Tenderloin, serving\na socially, economically, ethnically, and culturally\ndiverse patient population.\nA typical day begins around 6:30 AM.\nResidents gather in the resident lounge\nto receive sign out from the night team.\nWe don't have 24-hour call thanks to our dedicated\ncross-cover ICU and overnight admitting teams.\nOur intern-run cross-cover system\nprovides valuable experience in triaging and managing\ncomplex overnight issues with continuous support\nfrom in-house hospitalists and ICU attendings.\nYOHANNA KHACHIYAN: Here at CPMC, we\nhave daily morning reports led by the chief residents\nand a robust noon conference series\ncovering the full spectrum of Internal Medicine\nsubspecialties, as well as topics\nin neurology, ophthalmology, psychiatry, podiatry, and more.\nOLIVIA WANG: We focus on creating a collaborative\nand interactive learning environment where\nresidents and medical students develop\nevidence-based clinical decision-making skills.\nResidents work closely with dedicated teachers and mentors,\nincluding our hospitalists, cardiologists, intensivist,\ngastroenterologists, and transplant specialists,\nmany of them who trained here at CPMC.\nAnother strength of our program and what makes\nour community so remarkable is our exceptional ancillary staff.\nOur nurses, pharmacists, social workers, phlebotomists,\nand therapists are integral members of the care team.\nOur ICU rounds are highly collaborative,\nwith pharmacists, dietitians, and nurses\nworking alongside physicians to care\nfor critically ill patients.\nPRIYANKA SHINDGIKAR: We'd also like\nto highlight a few important locations.\nHere we have the Chuck Williams Cafe\nwhere residents receive complimentary meals while\non service and during clinic.\nWe are incredibly well fed.\nYOHANNA KHACHIYAN: Our busy emergency department\nincludes 39 more than 7,000 kidney, 2,100 liver,\nand 800 heart transplants.\nResidents also gain exposure to our cardiac catheterization\nand interventional radiology services,\nas well as our internationally recognized\ninterventional gastroenterology program.\nOLIVIA WANG: For our categorical residents,\nwe place a strong emphasis on primary care.\nLocated in Pacific Heights, our resident clinic\nserves as a cornerstone of comprehensive, patient-centered\ncare.\nResidents build their own patient panels\nand develop the skills needed to become outstanding primary care\nphysicians.\nYou'll be part of a close-knit clinic\nteam led by an outpatient attending\nwho provides individualized mentorship and teaching.\nOur primary care curriculum emphasizes\nevidence-based medicine and quality improvement.\nWe serve a diverse patient population\nwith commonly spoken languages including Cantonese, Russian,\nSpanish, Taishanese, and Tagalog.\nOur clinic also has a dedicated social worker and case manager\nto help with our patients' complex needs\nand address the social determinants of health.\nPRIYANKA SHINDGIKAR: We use a 4 plus 2 or x\nplus y curriculum where categorical residents spend\nfour weeks on inpatient services,\nfollowed by two weeks in outpatient rotations\nor electives.\nPreliminary interns do not participate in primary care\nrotations unless they've chosen to do one as an elective.\nYOHANNA KHACHIYAN: Our preliminary interns\nare fully integrated members of the CPMC community.\nAlthough they spend only one year with us,\nthey often form meaningful relationships\nand lifelong friendships.\nOLIVIA WANG: Congratulations on your outstanding personal and\nprofessional accomplishments.\nPRIYANKA SHINDGIKAR: Your talents, dedication,\nand potential were evident throughout your application.\nYOHANNA KHACHIYAN: We truly look forward\nto virtually meeting each and every one of you.\n[MUSIC PLAYING]"}
