Yumi DiAngi Taylor, M.D., FACP

Dr. Yumi is a board-certified internal medicine and clinical informatics physician, and a founder of Tera. She received her medical degree from the University of Pennsylvania School of Medicine and worked for six years in rural Pennsylvania, where her practice was recognized as a five-star quality clinic by Highmark BCBS and CMS (Center for Medicare Services). She recently completed a clinical informatics fellowship at Stanford, where she learned the ways data and technology can deepen relationships and improve patient health. Yumi is a voracious reader (and audiobook listener) who enjoys running, swimming and playing board games with her two children in her spare time. In addition to English, Yumi speaks conversational Japanese.

Arlene Sugatan, LVN

Arlene received her Associate Degree in Guam, where she worked as a medical assistant for 2 years before moving to California. In her free time, Arlene enjoys reading, cooking and traveling with her son. She is fluent in both English and Tagalog.

Set up your account.

New patients must download and enroll in My Health Online to contact the care team and schedule appointments. My Health Online is a digital platform where you can have video visits with your care team, send messages and access health information (such as labs and appointments).

  • 1. Enroll in My Health Online Enroll through our website, text chat or by calling (866) 978-8837
  • 2. Activate your Account Use your unique activation code to sign in to My Health Online on your web browser. You will be prompted to create a username and password.
  • 3. Download the My Health Online mobile app The app is available at the Apple App Store and Google Play. Log in to your account and set up notifications to see if you have appointments, messages or payments.

  • 1. Check My Health Online messages. You will receive a My Health Online message with a link to the new patient questionnaire for Tera.
  • 2. Click the link. The link will take you to a secure site where we ask you to answer questions about your health and life. The questions save as you go, so you can finish it all in one sitting, or little by little. We ask that you complete your questionaire at least 24 hours before your video visit, so that the Tera team has time to review your responses before you meet.

Meet your health team.

Each new patient must schedule a virtual Doctor Meet & Greet (there is no copay for the visit). This video chat is a chance for you to meet your primary care physician and other members of the Tera team. Together, we review your health snapshot, discuss medical needs and talk about your goals.

  • 2. Navigate to the appointments page, tap on your scheduled visit, and then press the green video icon to begin.
  • 3. Meet directly with your care team (including your doctor and nurse), skipping the commute and waiting room.

Frequently Asked Questions

Tera is a medical practice led by Yumi DiAngi Taylor, M.D., which provides a flexible alternative to primary care. The care team includes Dr. Yumi, plus a health coach and a nurse. You can expect the same services you receive from your PAMF primary care provider, plus additional benefits like flexible scheduling, personalized coaching, video visits and on-the-go messaging. This access can reduce the need for in-person appointments because you have a care team to answer questions and treat symptoms as they arise.

Plus, the Tera care team acts as a single point of contact to address your questions, resolve health problems, review lab results and help you manage medications. This team helps you schedule appointments with specialists and works closely with those doctors to make sure your needs are met. We also communicate with the people involved in your healthcare, including family members, caregivers or other providers you give us permission to share with. With Tera, you have flexible access to your care team through Sutter’s My Health Online (MHO) app. MHO can be used to send private messages, access digital tools and conduct video visits.

Additionally, our virtual-first model means that we can provide longer appointments when you need it, and quick ones when you don't. Many patient needs can be met through phone, email, or video visits, and in-person appointments can be easily scheduled when needed.
If you have an issue that requires in-person care, the team will connect you with Dr. Yumi or an appropriate PAMF provider. We’ll set up the appointment for you to ensure seamless coordination and follow-up.
Please fill out the Contact Us form. If you are eligible for Tera, the team will follow up via phone or email. Once you enroll in Tera, we will send you an intake survey, followed by a Doctor Meet & Greet video call. This is a chance for us to get to know you and understand your health needs.
We believe that good healthcare should be accessible to everyone, and we built a model to match. This expanded access is provided at no additional cost. Your Doctor Meet and Greet video visit does not have a co-pay. Any visits that can be resolved via email or phone call have no co-pay. Depending on your health plan, there may be a co-pay for video visits, just as there are with office visits.

For additional questions about copay and insurance reimbursement, your insurance company is usually the most knowledgeable. Their con-tact information is printed on your insurance card.

A cell phone or laptop is all you need to access video appointments and My Health Online. Through the My Health Online app, you can email your care team, see lab results, request prescription refills and make appointments. Much of our communication happens via the My Health Online app, so we ask that patients consider their comfort with mobile applications when considering becoming a patient.
As much as they can, our care team will troubleshoot initial technical issues with telemedicine, My Health Online and other digital tools. For specific questions about My Health Online, you may also contact our support team any time.
