Description
To develop and pilot test an electronically delivered Cardiometabolic Program (eCMP), based on the in-person CMP founded in 2004 by Tak Poon, M.D., PharmD, ABIHM, FACC. CMP is a 4-6 month intensive lifestyle and stress management group coaching program for obese patients with established cardiometabolic morbidities (e.g., type 2 diabetes mellitus and coronary heart disease). Results from the pilot study will guide continuing refinement and implementation of eCMP, which is targeted for high-risk primary and secondary prevention adult patients who can benefit from intensive, practical and sustainable lifestyle management to reduce cardiometabolic risk.
