My Health Online Help Your Teen Enroll

Are You Eligible?

Teens must have their own My Health Online account in order to view all of their health records.

  • Teens age 12-17 can enroll in My Health Online with parental permission.
  • The teenager and parent must both be Sutter Health patients.
  • Want to become a patient and sign up for an MHO account? Find a Sutter provider who offers My Health Online and you can enroll during your first visit.

How to Enroll

Choose the situation that fits you and follow the steps for enrollment.

    • Sign in to your My Health Online account.
    • View My Family's Records.
    • Tap Authorize Teen with My Health Online, then follow the instructions.
    • After consent is established, teens can set up a personal My Health Online account at any doctor’s visit. Or they can call (866) 978-8837 to activate their account.
    • Sign in to My Health Online.
    • View My Family's Records.
    • Tap Add New, then follow the instructions.
    • After parental or guardian consent is granted, teens can set up a personal My Health Online account at any doctor’s visit. Or they can call (866) 978-8837 to activate their account.
    • Visit Proxy Access for Children and follow the instructions to establish proxy access to the teen’s account.

  • Enrollment for teens can only be done in person when parents or guardians don’t have their own My Health Online accounts. At the teen’s next doctor visit, inform the care team the teen would like to open a My Health Online account and that you give your permission.

  • You can complete your enrollment online now. Activate your account.
