My Health Online Get Proxy Access for Adults

Manage and view the health records of adults age 18 and older.

You can sign up for online access to an adult’s health record if:

The adult patient is age 18 or older.
The patient is a family member or is under your care.
If neither you nor the patient have My Health Online accounts, fill out and sign the adult proxy authorization form.

If you have a My Health Online account, follow these steps to give access to another adult:

Sign in to your My Health Online account and select Sharing Hub from the main menu.
Select Give Friends and Family Access and tap Invite Someone.
Fill out the form and send the invite.
The person you invited will receive an invitation to access your account via email.
Once they tap Accept Invite, the invitee has to sign in to MHO and enter your birthdate.
Once they’ve accepted, they’ll be able to view your health records.

Fill out an authorization form. Submit the completed and signed form by:

You’ll receive an access code and sign-up instructions via email.

Patients who wish to enroll in My Health Online can submit a Release of Information request (ROI) to establish an account.
