Sutter Care at Home Awarded CHAP Accreditation for Home Health and Hospice Programs in Northern California
Community Health Accreditation Partner, Inc. (CHAP) announced today that Sutter VNA and Hospice (DBA Sutter Care at Home) has been awarded three-year accreditation under the CHAP Home Health and Hospice Standards of Excellence, effective Nov. 4, 2015. By achieving CHAP Accreditation, Sutter Care at Home has also been deemed to meet the Medicare Conditions of Participation and is certified as a Medicare provider.
CHAP Accreditation demonstrates that Sutter Care at Home meets the industry’s highest nationally recognized standards. The rigorous evaluation by CHAP focuses on structure and function, quality of services and products, human and financial resources and long term viability. Simply stated, adherence to CHAP’s standards leads to better quality care.

In addition to the CHAP Core Survey accreditation, Sutter Care at Home locations in San Mateo, Modesto, Marin and Santa Cruz were deemed to meet Medicare Conditions of Participation by the Centers for Medicare and Medicaid Services (CMS). The remaining home health and hospice locations will undergo the deemed and accreditation surveys in the next few months.
