# Proxy access

## Proxy access in My Health Online

Proxy access lets parents and caregivers view health information and help manage care for someone else through My Health Online.

Choose the option below that fits your situation:

Access to your dependents’ My Health Online accounts can help make managing their care easier.

View their electronic medical records

Review medications or request renewals

View test results, when available

Schedule medical appointments

Send messages to their care team

View letters from Sutter Health staff

Proxy access for adults

Allow proxy access to your healthcare record through My Health Online.

You can allow online access to your health record if:

-   The person you're allowing access is age 18 or over.
-   The adult you're inviting is a family member or has a legal right to manage your care.
-   If you want to provide access to your account to someone who isn't a Sutter patient, fill out and sign the [adult proxy authorization form](https://edge.sitecorecloud.io/sutterhealt962c-sutterhealt8fce-production57cc-4860/media/Project/SutterHealth/SutterHealth/Files/PatientResources/proxy-access-adult.pdf "adult proxy authorization form").

**Allow Proxy Access in My Health Online** 

If you have a My Health Online account, follow these steps to give access to another adult:

1.  Sign in to your [My Health Online](https://myhealthonline.sutterhealth.org/mho/Authentication/Login?) account and select **Sharing Hub** from the main menu.
2.  Select **Manage Friend and Family Access** and tap **Invite Someone**.
3.  Fill out the form and send the invite.
4.  The person you invited will receive an invitation to access your account via email.
5.  Once they tap **Accept Invite**, the invitee has to sign in to MHO and enter your birthdate.
6.  Once they’ve accepted, they’ll be able to view your health records.  
     

**Allow proxy access via authorization form**

Fill out this [authorization form](https://edge.sitecorecloud.io/sutterhealt962c-sutterhealt8fce-production57cc-4860/media/Project/SutterHealth/SutterHealth/Files/PatientResources/proxy-access-adult.pdf "authorization form") to request or allow proxy access if:

1.  You're an adult who wants to grant access to your health records without enrolling in My Health Online.
2.  You're a guardian or power of attorney requesting access for another adult's account.

Print, complete and sign the form. Email it to [MHOsupport@sutterhealth.org](mailto:MHOsupport@sutterhealth.org) or mail it to the address on the form.

_Patients who wish to enroll in My Health Online can submit a_ [_release of information request_](https://www.sutterhealth.org/patient-resources/my-health-online/-/media/Project/SutterHealth/SutterHealth/Files/PatientResources/sh-enrollment-form.pdf) _to establish an account._

Proxy access for teens ages 12–17

Teens have their own My Health Online accounts. Parents or guardians can still request proxy access, but what you can see is limited under [California privacy laws.](https://www.chhs.ca.gov/wp-content/uploads/2023/04/State-Health-Information-Guidance-5.1-2023.pdf "California privacy laws.")

When a child turns 12, their account transitions to teen proxy settings.

To request proxy access:

-   The teen may obtain [their own My Health Online account](https://www.sutterhealth.org/patient-resources/my-health-online/teen-enrollment)
-   You’re legally allowed to access the teen’s health information and make medical decisions
-   You and the teen live at the same address (or provide additional documentation if you don’t)

**Request Proxy Access in My Health Online**

Sign in to your [My Health Online](https://myhealthonline.sutterhealth.org/mho/inside.asp?mode=showform&formname=SHCheckProxy "My Health Online") account.

1.  Select **Add Access for Parents and Caregivers** from the main menu.
2.  Enter in the child's name and date of birth.
3.  Click **Request Access**.

**Request Access Via Authorization Form**

Submit a [proxy access authorization form](https://edge.sitecorecloud.io/sutterhealt962c-sutterhealt8fce-production57cc-4860/media/Project/SutterHealth/SutterHealth/Files/PatientResources/proxy-access-child.pdf "proxy access authorization form") to request access to a minor’s records if:

-   You’re the parent or guardian of a child who’s a Sutter patient but you don’t have your own My Health Online account.
-   You don’t share the same home address as the child for whom you’re requesting proxy access.
-   Print, complete and sign the form. Email it to [MHOsupport@sutterhealth.org](mailto:MHOsupport@sutterhealth.org) or mail it to the address on the form.

**Help Your Teen Enroll in My Health Online**

Teens can create their own My Health Online account to view their health information and manage their care. Get [step-by-step instructions](https://www.sutterhealth.org/patient-resources/my-health-online/teen-enrollment) on how to enroll.

Proxy access for children age 11 and younger

With proxy access in My Health Online, you can view and manage health information for a child age 11 and younger.

You can request proxy access if:

-   You’re the child’s parent or legal guardian
-   The child is age 11 or younger
-   You’re legally allowed to access the child’s health information and make medical decisions
-   You and the child live at the same address (or provide additional documentation if you don’t)
