How to enroll Choose the situation that fits you and follow the steps for enrollment. **I’m a parent or guardian with my own My Health Online account. How can my teen open their own MHO account?**  1. Sign in to My Health Online. 2. View [My Family's Records](https://myhealthonline.sutterhealth.org/mho/Authentication/Login?postloginurl=inside%2Easp%3Fmode%3Dshproxylist). 3. Tap **Add New**, then follow the instructions. 4. Teens can set up their own My Health Online account at any doctor’s visit. Or they can call [800-4SUTTER](tel:18004788837) to activate their account. 5. Visit [Proxy Access](https://www.sutterhealth.org/patient-resources/my-health-online/proxy-access) and follow the instructions to establish proxy access to the teen’s account. **I’m a parent or guardian but I don’t have my own My Health Online account. I’d like to help a teen establish their own My Health Online account.**  Enrollment for teens can only be done in person when parents or guardians don't have their own My Health Online accounts. At the teen's next doctor's appointment, inform the care team your teen would like to open a My Health Online account.  **I’m a teenager between ages 12-17.
