# Sutter Care Management

Managing your health can take more than office visits. Sutter Care Management connects you with a dedicated virtual care team that works with your primary care provider to help coordinate your care and support your health and wellness goals.

## What is Sutter Care Management?

Sutter Care Management is a no-cost, personalized care program for patients who may benefit from extra support between visits. The program connects you with a dedicated virtual care team that works alongside your primary care provider to help coordinate your care, answer questions, and support your health and wellness goals.

Depending on your needs, your team can help you better understand your care plan, manage medications, prepare for appointments, connect with resources and stay on track. 

**Your care team may include:**

Nurses

Social Workers

Health Care Coordinators

Chaplains

Pharmacists

Dieticians
