Sperm Donation Services at PAMF Fertility

If sperm donation becomes part of your family planning, PAMF medical staff will provide detailed information and help you through the process. You’ll benefit from our extensive experience working with sperm banks and known sperm donors.

Sperm banks must comply with rigorous regulatory requirements from the State of California and the FDA. PAMF Fertility recommends the following sperm banks that meet both FDA and state requirements.

To Order Donor Sperm

Due to California tissue bank licensing requirements, we only accept specimens from the sperm banks listed here. These sperm banks have profiles containing detailed information available for your review. Costs vary depending on the facility. It takes at least three business days to ship sperm. Overnight Federal Express delivery is available. Please follow these steps when ordering donor sperm:

1. It is very important to place your order prior to starting a cycle. Contact the lab or your coordinator for the necessary forms.
2. Obtain a physician’s order to order donor sperm.
3. Sign a Semen Storage Agreement in front of FRHI/FPNC representative (notarized signature OK).
4. Send a Donor Sperm Order Form to the lab every time a new shipment is coming. If we don’t have this form on a delivery day we will decline the delivery.
5. Please order a minimum of two vials and not more than six vials.
6. Please have the donor sperm delivered to:

Deliveries are only accepted Monday through Friday, from 8:00 a.m. to 2:00 p.m. We cannot accept weekend deliveries.
