## Why Choose Sutter?

Heart failure affects nearly 7 million Americans. The condition occurs when the heart can no longer pump enough blood to meet the body's needs. Several different heart-related problems can cause heart failure, and it can become a chronic, lifelong condition.

Let us protect your heart. Our experienced heart failure specialists have successfully treated heart failure for decades. When you have acute or chronic heart failure, you’ll benefit from a team of specialists who collaborate closely on your care and progress every step of the way. Our teams of highly skilled cardiologists, advanced heart failure specialists, pharmacists, social workers and dietitians will help you manage heart failure with a range of treatments. If you need a heart transplant, we’ll support you through diagnostic testing, surgery and cardiac rehabilitation. Our care nurses are also available so you can receive online consultation, support and important information from the comfort of your home.

## Special Programs

**Free heart failure support**

You don't have to manage heart failure on your own. Sutter offers free programs that connect you with pharmacists who specialize in heart failure and provide one-on-one support between visits with your cardiologist.

Sutter Sync is Sutter's primary heart failure support program, offering ongoing support and home monitoring for eligible patients. Patients who aren't eligible for Sutter Sync may be referred to VPACC-HF, which provides short-term support until medications are optimized and symptoms are stable.

Whether you're enrolled in Sutter Sync or VPACC-HF, our specialized pharmacists can help you:

- Review and optimize your medications, including finding lower-cost options
- Understand how to take medications safely
- Learn how food, sodium, and fluids can affect heart failure
- Recognize signs that your condition may be changing

**Sutter Sync: Specialized heart monitoring and care**

Sutter Sync is a free program that helps you track your heart health from home and stay connected to care between visits with your cardiologist.

You'll receive ongoing support from a pharmacist who specializes in heart failure, along with easy-to-use monitors that automatically send readings to your My Health Online (MHO) health chart. Your pharmacist reviews your readings, helps you understand your trends, and adjusts medications when needed.

### Once enrolled, you’ll get:

- One-on-one support from a heart care pharmacist
- A free blood pressure monitor and Bodyport® Cardiac Scale that helps track changes in fluid levels
- Support for Type 2 diabetes and cholesterol, if needed
- Monthly summaries of your heart health
- Personalized health tips in MHO

Sutter Sync is designed for long-term support. All that's required is your participation — take readings at home and keep in touch with your pharmacist to make sure your treatment plan is working well for you.

### Who’s eligible?

Sutter Sync is available to patients who:

- Are enrolled in an eligible Medicare Advantage, MSSP or SutterSelect plan
- Have a smartphone or tablet with the My Health Online app
- Have a heart failure diagnosis
- Are 18 years or older
- If you're eligible, you'll receive an invitation through your My Health Online (MHO) message center.

### Questions? Give us a call.

If you'd like to talk with someone before enrolling, call the Sutter Sync Care Team at [855-320-5200](tel:+8553205200). We're available Monday through Friday, 7:00 am – 7:00 pm (PT), and can answer your questions, explain how the program works, and help you decide if it's right for you.

Learn more at [www.sutterhealth.org/sync](https://www.sutterhealth.org/patient-resources/my-health-online/sync)

**Virtual Pharmacy Ambulatory Care Clinic for Congestive Heart Failure (VPACC-HF)**

VPACC-HF is available for patients who aren't eligible for Sutter Sync. The program provides short-term support to help patients reach a stable treatment plan. Through scheduled phone appointments, pharmacists who specialize in heart failure provide medication support, education, symptom monitoring and personalized guidance. There’s no cost to participate.

Depending on your needs, your pharmacist may recommend changes to your medications, daily weight tracking, sodium and fluid limits, or other steps to help manage your heart failure.

- To get the most from the program, we ask patients to:
- Keep an open mind and be willing to make changes to medications or daily habits
- Focus on realistic goals that can help stabilize symptoms and improve quality of life
- Participate in scheduled phone appointments and discuss any changes in your health
- Let us know in advance if you need to reschedule an appointment
- Seek medical care if you notice worsening symptoms or increasing discomfort related to your heart failure

**Virtual Pharmacy for Congestive Heart Failure**

Our Virtual Pharmacy Ambulatory Care Clinic (VPACC) for Congestive Heart Failure (CHF) program is staffed by a team of specially trained pharmacists who work closely with cardiologists. The program is designed to help CHF patients manage their condition and improve their quality of life. VPACC has seen a 45% reduction in one-year mortality rates and a 60% decrease in cardiac-related hospitalizations since its launch in 2023.

Pharmacists serve as a resource for patients — [referred by their physician](https://www.sutterhealth.org/healthcare-professionals/how-to-refer-vpacc) — to learn more about their heart disease and understand their medications. There is no cost or insurance needed to enroll in this telephone support program.

### How It Works

Pharmacists review a patient’s health history, medications and current home management, and offer a personalized assessment.

Pharmacists will also:

- Educate patients about Congestive Heart Failure (CHF)
- Help patients recognize signs and symptoms of worsening CHF and know when to get help
- Optimize medication treatment
- Provide education on how they work, how to take them, and how to identify possible side effects
- Help review the costs of their medications and find out if they can switch to a lower-cost version from their pharmacy
- Order new heart failure medications or adjust medications to the right dose so patients can live longer and healthier lives
- Help patients understand how eating food with too much sodium and drinking too much liquid can worsen CHF
- Describe the importance of daily weigh-ins and proper weighing techniques
- Help patients take care of themselves and manage their condition

Certain suggestions may be made, including a change in current medication regimen, recording daily weights, sodium and fluid restrictions, and medication adherence.

We ask patients to:

- Keep an open mind, and be willing to make changes (e.g., medications, lifestyle)
- Achieve realistic goals. We won’t be able to “cure” heart failure, but we will take steps toward stabilizing their condition and improving their overall quality of life
- Be present and ready to discuss their health at our scheduled phone appointments
- Let us know in advance if they will not be available and need to reschedule
- Seek medical evaluation at signs of worsened congestion or discomfort related to your heart failure

The Virtual Pharmacy team maintains continuity with the patient’s primary care provider and cardiologist to provide the best care possible.
