You and your family can schedule a free, one-hour consultation with an insurance counselor at select locations. Counselors can answer questions about Medicare, supplemental insurance, HMOs and long-term care coverage. They can also help you choose the best supplemental insurance for your needs and set up a system to simplify filing claims. Talk with your care team about receiving insurance counseling. **Program of All-Inclusive Care for the Elderly (PACE)** The [Program of All-Inclusive Care for the Elderly (PACE)](https://www.sutterhealth.org/services/sutter-seniorcare-pace) is a nonprofit health plan designed for frail older adults. PACE provides high-quality, comprehensive services to anticipate potential issues and prevent the need for nursing home care. It offers coordinated care through a wide range of services, with benefits covered by Medicare and Medi-Cal. Most care is delivered at PACE centers by an interdisciplinary team, including doctors, nurses, social workers and rehabilitation therapists. Speak with your care team to learn more about receiving care at a PACE center near you. **Adult Day Health Care** Our Adult Day Health Care programs support your independence through activities, nursing care and therapies like occupational, speech and physical therapy. Available at select Sutter locations in Northern California, these programs aim to improve physical and mental skills while offering social interaction to help prevent loneliness. We closely monitor your health and medications to ensure you get the care you need. Depending on your needs, you can attend two to five days a week, and some locations offer special programs for those dealing with memory loss. Talk with your care team to learn whether there’s a program near you. **Sutter Safe at Home** Feel confident and safe at home or on the go. Sutter Safe at Home is a personal emergency alert device program that helps connect people who have fallen to emergency services. A help button worn as a bracelet or necklace allows you to easily communicate with our response center. We also provide free lock boxes, allowing first responders to access your home safely and without force. We offer two device options, HomeSafe and GoSafe, based on your lifestyle and personal needs. Highlight features include: - Receive 24/7 help. - Wear your waterproof help button as an adjustable necklace or on your wrist like a bracelet or watch. The help buttons are designed to be comfortable, and the neck cords are designed to break away in an emergency situation. - Auto fall detection can be programmed into your device at no additional charge.
