Description

The purposes of this collaboration: 

(1) Develop and pilot test of a digital health solution called CM-SHARE. 

(2) Develop scalable methods for management of patients with cardiometabolic disease.  It includes developing and testing new approaches to identify clinically meaningful segments of patients, and to identify and close health care gaps through segment-tailored models of care.
