Advance Care Planning in Older Adults With Multiple Medical Conditions Undergoing High-Risk Surgery

Description

High-risk surgery in older adults with multiple comorbid conditions (MCC) leads to major life-altering complications for at least 1 in 5 patients. In order to prepare patients and families for these adverse outcomes, advance care planning (ACP) must occur in the pre-operative setting. Little is known about ACP among older adults with MCC.

The goal of this pilot project is to determine the prevalence of pre-operative documentation of ACP among older, high-risk surgical patients with MCC and the patient characteristics associated with ACP documentation.
