Sutter Care at Home provides compassionate, personalized care in the comfort of a patient's home. Our visiting nurses, physical therapists, speech therapists, occupational therapists, medical social workers, dietitians, and other health professionals work closely with physicians to deliver exceptional, comprehensive home healthcare. Our ability to provide nationally-recognized quality care is possible not only because of our highly skilled clinical staff, but also because of the generosity of individuals who partner with us.

Hospice
Sutter Care at Home’s Hospice program provides physical, spiritual and emotional support focusing on pain management and symptom control, as well as comforting services and therapies.
Philanthropy supports numerous areas within the Sutter Care at Home Hospice program:
- Volunteer Training: providing in-person and web-based training that directly impacts the volunteer’s ability to support our hospice patients and their families.
- Translation Services: empowering communication and understanding among non-English speaking patients and families regarding important matters of care.
- Alternative Therapies: combining traditional care with complementary therapies such as massage, guided imagery and aromatherapy as part of our patient-centered approach.
- Music and Memory: offering comfort to patients and awakening past memories through the use of familiar playlists and volunteer support.
Your gift to Hospice will allow our trained team of clinicians and volunteers to provide a range of supportive therapies to our patients and their families.
Bereavement
The Sutter Care at Home Grief and Bereavement Support program is open to the public and not just to those who use our hospice services. The program provides individual, group and school-based bereavement therapy services to more than 3,000 adults and 4,000 children each year. The Bereavement and Grief Support program includes a strong volunteer component with over 600 volunteers contributing 77,000 hours a year.
Sutter Care at Home leaders are committed to growing our Bereavement and Grief Support Programs in 2019 and beyond. This will not only serve the increasing number of hospice families who look to our services as part of their overall benefit, but it will also allow us to support expansion of our community based outreach.
Your gift to bereavement programs will expand our services, add additional staffing, and increase the number of community based clients and services we support.
Home Health
Our Home Health service line includes care for homebound patients, home infusion therapy, curative treatment, home visits for rehabilitation, and speech and occupational therapy. Sutter Care at Home serves more than 30,000 Home Health patients each year.
Today’s home health patient has more complicated medical needs than ever before - needs which would have required continued hospitalization just a few years ago. As a result, Home Health Case Management has become an identified strategic priority for Sutter Care at Home.
Your gift to Home Health will assist in creating a sustainable model for Case Management that can effectively:
- Improve patient, family caregiver, and staff satisfaction
- Reduce emergency room visits, hospitalizations, and adverse health events among SCAH Home Health patients
- Ensure clinicians have the information and support they need to make informed decisions across the continuum of care
