If you haven’t been contacted by our insurance verifier within three days of your appointment, please contact the billing office at 855-215-4614, option no. 1, for information regarding your insurance coverage and possible co-pay required at registration.

If you’re calling regarding a procedure that has already taken place and have a question about your bill, please contact the billing office at 916-596-2981.

If you have any questions about your anesthesia billing, please contact the billing office at 408-333-9108.

Your ride may drive up to the front of our center to drop you off and pick you up. A white zone is designated for the center. A nurse will call your ride approximately 30 minutes before you are ready for discharge.

Please bring the following items to your appointment:

• History and physical questionnaire form (found in Resources and Forms)
• Insurance card(s)
• List of current medications form (found in Resources and Forms)
• One form of a valid photo ID
• Payment for any co-pays or deductibles
• If you are a legal guardian or conservator of a patient, please bring all legal paperwork with you, including the Durable Power of Attorney for Healthcare.

Your ride may drive up to the front of our center to drop you off and pick you up. A nurse will call to inform you of the patient pick-up time approximately 30 minutes before they are ready for discharge.
