As a member of the PAMF hospital team, I provide support to patients following a stay at El Camino Hospital. My goal is to facilitate the transition home and ensure patients, family members, and caregivers understand the entirety of the discharge plan. This includes medications (new prescriptions and changes to existing medications), follow-up tests such as blood work, as well as follow-up appointments with primary care providers and specialists. To this end, I visit patients at El Camino Hospital toward the end of their stay, perform follow-up phone calls shortly after discharge, and often complete a single home visit the following week. My goal is to facilitate a smooth transition to the outpatient setting for patients recovering from a hospital stay.
