PAMF Community Health Resource Centers (CHRC)

PAMF has Community Health Resource Centers (CHRC) in Danville, Dublin, Fremont, Mountain View, Sunnyvale, San Carlos  and Palo Alto. Staffed by health educators, the centers are open to PAMF patients and other community members. Services are provided at no cost. We can assist you in finding information and resources to help you make informed decisions about your health and wellness.
