My Health Online Get Proxy Access for Kids

Manage and view the health records of children age 17 and under.

You can sign up for online access to a minor child’s health record if:

You’re the child’s parent or guardian.
The child is age 17 or under. Note that teens age 12-17 can have their own accounts.
You’re legally allowed to access information and make medical decisions for the minor.
You and the child live at the same address.

Follow these steps if you have an MHO account.

Submit a proxy access authorization form to request access to a minor’s records if:

You’re the parent or guardian of a child who’s a Sutter patient but you don’t have your own My Health Online account.
You don’t share the same home address as the child for whom you’re requesting proxy access.

You’ll receive an access code and sign up instructions via email.
