Notice of Intended Supplemental Service Closure

California Pacific Medical Center – Van Ness Campus Outpatient Service – Laboratory at 3838 California Street, Suite 106, San Francisco, will close no sooner than May 23, 2025.  All Laboratory services provided at the 3838 California Street location will be consolidated to other nearby California Pacific Medical Center locations and a newly opening Sutter Pacific Medical Foundation location in the same building.
The nearest available lab facilities are at the following locations:

• Sutter Pacific Medical Foundation Laboratory, 3838 California Street, 4th Floor (Opening May 27, 2025).  Medicare and Medi-Cal will be accepted at this location.
• CPMC Pacific Heights, 2100 Webster, Suite 103, San Francisco, located 1.5 miles away.  Medicare and Medi-Cal are accepted at this location.
• CPMC Van Ness Campus Medical Office Building, 1100 Van Ness Avenue, 2nd Floor, located 2.3 miles away.  Medicare and Medi-Cal are accepted at this location.
• CPMC Davies Campus Hospital, 601 Duboce Avenue, A-Level, located 2.4 miles away.  Medicare and Medi-Cal are accepted at this location.
