At Sutter Tracy Community hospital, our overall goal of our rehabilitation department is to deliver comprehensive services to individuals including infants, children, adolescents, adults and seniors. Our licensed therapists evaluate patient function, determine goals of therapy, and perform and develop a personal treatment plan in order to obtain a positive outcome whether it is to return to work or to a productive living. Inpatient and outpatient services are provided and include comprehensive assessments, treatment planning, delivery and documentation of care, consultation, re-evaluation and discharge planning. All procedures require a written order from a licensed physician.
Services are provided with emphasis on quality patient care based on sound professional ethics. Diagnostic assessment, direct therapeutic intervention, patient and family/caregiver education and staff consultation and training are integral portions of the Physical Therapy services program.
Inpatient services are performed at the bedside on all acute care units. Outpatient services are provided in the rehabilitation clinic.
