Coming Home Hospice - CPMC

Our volunteers meet and greet family members, answer phones, and spend time with patients. Please complete the application form if you are interested in becoming a California Pacific Medical Center Volunteer Services volunteer. You must be 18 yrs. old and above to complete the form. Once you complete the form, click the submit button at the bottom. Privacy Policy information regarding our website can be viewed by clicking the words Privacy Policy located at the bottom left corner of the application.

Once submitted, applications are reviewed on the first week of the following month.

• Ages 18 and older.
• Commitment to serve a minimum of 100 hours in one year, at least three to four hours per week.
• Complete online application and in-person interview.
• Complete and pass a background check (adults only).
• Complete and pass a health screening.
• Junior Volunteers — have signed parental consent form.
