How to Apply
Program Eligibility
- Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited Doctor of Pharmacy degree program or have a Foreign Pharmacy Graduate Examination Committee (FPGEC) certificate from the NABP.
- Must be licensed or eligible for licensure in California.
- US Citizen or Permanent Resident
Application Requirements
- Letter of intent outlining career goals, areas of interest, and reasons for applying to the residency program at Memorial Medical Center, Modesto
- Official academic transcript
- Curriculum vitae
- Three letters of recommendation – We recommend that at least one letter should be from a clinical preceptor (e.g., an acute care or ambulatory care rotation)
