- Graduation from an accredited pharmacy school
- US citizen or permanent resident
- Eligible for California pharmacist licensure

Applicants must submit a standard application through the Pharmacy Online Application Service (PhORCAS) application with the following information:
- Statement of Interest/Letter of Intent
- Official academic transcript
- Curriculum vitae with match number
- Three reference writers utilizing the PhORCAS form
- Supplemental application - Upload in PhORCAS a PDF file answering the following:
- Describe an important aspect of your personal background that will give us insight into you as an applicant. Aspects might include, but are not limited to, significant challenges or obstacles you faced and how you overcame the challenge, a situation that motivated you to evaluate your strengths and weaknesses, or how you navigated an unexpected outcome and what you learned from the experience. (Maximum 500 words)
All application requirements must be submitted via PhORCAS by application deadline, January 3rd, 2025, to be considered for an interview.
