Initial Service Request
Our program allows you to reach out for support from a Licensed Mental Health Therapist trained to help you find solutions including work-related problems, mental health issues, marriage problems, financial consultations and child care referrals.

Renew Service Request
At the beginning of each new benefit period, submit this form if you would like to continue with the same Licensed Mental Health Therapist for renewal authorization. If you wish to change your Licensed Mental Health Therapist, submit the initial request form to start the process.
