If payment isn’t received within 15 days, you’ll receive a second notice. This is the final request for payment. This notice will include the retroactive coverage termination date, or the date when the premium was originally due. You may be responsible for repaying claims made after the coverage termination date. If you return to work with an outstanding balance still due, you must pay this premium or coverage will be terminated even if current premiums are being deducted from your paycheck.
If coverage is terminated due to nonpayment of premiums, you and any covered dependent(s) won’t be eligible to continue coverage through COBRA. You may enroll in benefits during the next open enrollment period. There is one exception: If you’re returning from a protected leave of absence (FMLA, PDL, CFRA), you can restart your coverage effective on the first of the month following election. To do this, you must re-enroll and complete the enrollment process within 60 days of your return to work. This exception doesn’t apply if you’re returning from any other type of leave of absence.
