How to Refer to the Sutter Referral Navigation Team

Our concierge referral team is available to answer your questions. We collaborate with Foundation Centralized Referrals, Sutter Physician Services, medical staff of Sutter-affiliated hospitals, and Utilization Management to help pull together multi-disciplinary teams to treat patients.

Referral Methods
Once Sutter Health Referral Navigation Team has received all the necessary information, they will:
- Review the referral within 24-48 hours. Submit to the specialist for review. Make a welcome call to the patient. Email the patient requested information. Follow the referral through completion. Fax status updates to the referring office.
Please note: URGENT referrals have priority.
The Sutter Health Referral Navigation Team understands that timely access to care is critical. Our process is focused on patient care and aims to expedite referrals whenever possible.
Make a referral request to Sutter Health Referral Navigation Team through Epic (Sutter EHR) by selecting the following:
- Class: Outgoing
- Place of service/location: Sutter Health Referral Navigation Team
Make a referral request to Sutter Health Referral Navigation through Sutter Community Connect by selecting the following:
- Class: Outgoing
- Place of service/location: Sutter Health Referral Navigation
The PREFERRED METHOD is to be submitted via CERM (Care Everywhere Referral Management)
If your Health Care Organization participates in CERM, this is the preferred method for referral submission. CERM improves coordination by synchronizing referrals via orders with other organizations.
- Refer to our Places of Service as you normally would
- If your organization is linked to the correct Direct Address, your referral will be routed through the CERM workflow

If your office has an Electronic Health Record (EHR) system and your Health Information Service Provider (HISP) supports DSM, referrals can be sent via DSM.
To access the Sutter Health Referral Navigation Team via DSM.
- Select Outside Message
- Select the patient you wish to refer.
- In “Add Recipients” enter one of the following:
- Sutter Specialty Network
- Sutter Health Referrals address sutter_specialty_network47810139@hisp.ehr.sutterhealth.org
- In the subject line include “New Referral”
- In the body of the message, include:
- Any relevant details (e.g., location/provider preferences, special instructions)
- Your signature or Smart Phrase with a contact phone number for follow-up

To Send a referral to the Sutter Health Referral Navigation Team:
- Open the New Referral function (Activities > Referrals > New Referral).
- Complete the first page of the New Referral form (below) with the referral details (Priority, Type, etc.) as you normally would.
- In the Class field, select External.
- Primary procedure> enter 99199 for all new referrals
- Type: enter office services.
- In the Referral to section, complete the following fields:
- Select SUTTER HEALTH REFERRAL NAVIGATION TEAM in the Location/POS field.
- Select the Provider Specialty needed (i.e., Cardiology) in the Prov. Specialty field. You can type a partial match to search for or click the magnifying glass to see a full list of options.
- In the Notes field, add any Clinical history that is relevant to the referral, or any information that should be considered when facilitating the referral. (i.e., if you want a specific provider, note it here)
- Upload any supporting clinical information or insurance cards before submitting a referral.
- When complete, click the Request Referral button at the bottom of the form to submit the referral.
Once submitted, the Referral Navigation Team will locate an appropriate provider if one isn’t specified.
To initiate the referral process, simply complete the Sutter Health Referral Navigation Team and fax or email along with:
- Copy of insurance card
- Chart notes
- Reports
- Lab results
