Absolutely. If you'd like to talk with someone before enrolling, call the Sutter Sync Care Team at 855-320-5200. A member of our team can answer your questions, explain how the program works, and help you decide if it's right for you. Our team is available Monday through Friday, 7:00 am – 7:00 pm (PT).

Currently, the program is by invitation only. If you’re invited, you’ll get a message in your My Health Online message center.

Yes! When you enroll in Sutter Sync, you pick what you’d like help with. If you have a diagnosis on file in My Health Online, that condition will be available for you to enroll in. 

Yes, you can still enroll if you use a CGM. We’ll also ask you to use your Sutter Sync glucometer. This way, your readings can be shared directly with your care team through My Health Online.

When you sign up, we'll send you a Care Kit. It’ll have detailed instructions and one or more digital monitors, depending on the conditions you're tracking. These monitors connect to your My Health Online account and send your real-time health data to our care team. 

Your care team includes your pharmacist, support team, and you. Your pharmacist keeps an eye on your readings. They may reach out if something needs attention, like high or low numbers or medicine changes. Your support team is here to help with things like monitor setup or technical issues. 

Your doctor and specialists with access to your medical record can also see your trends. You play an important role, too. The more you participate, the more personalized your care can be.

You can see all your health information in Sutter’s secure digital patient portal, My Health Online. Sutter Sync connects with My Health Online, so your health information stays safe and in one place. To send readings from your Sutter Sync devices to your health chart, you’ll need the My Health Online app on your phone.

Yes, your health information is safe and secure. Sutter Sync uses FDA-approved medical devices and secure technology to transmit your readings. Your readings and any other information you share with us are stored in My Health Online, a trusted platform designed to protect your personal health information.

Yes. The program is staffed by Sutter Health Care Team members and designed to work closely with your providers. Sutter Sync is integrated with My Health Online, so your health information stays in one place. Your Sutter Health providers can see that you're enrolled in Sutter Sync and view your readings in your health chart.

Unlike most programs, Sutter Sync doesn’t have a specific end date. That’s because we know managing ongoing health issues doesn’t end in just a few weeks.

We know you’re busy, so Sutter Sync is designed to fit into your routine. Depending on your program, you may take readings daily or a few times a week, and your care team may check in from time to time to talk through your readings. You’ll also receive short, easy-to-read Health Guides in My Health Online (optional).
