A diabetes diagnosis can be scary, but it can also be the start of a journey toward better health. Sutter Pacific Medical Foundation’s network of doctors, nurses, dietitians and diabetes educators partner with you to manage diabetes and restore and maintain your health.
Leveraging the expertise of our multidisciplinary Diabetes Clinical Improvement Community, we use a team approach and best practices to address all aspects of diabetes treatment and management.
Our specialists coordinate care with your doctor to ensure you receive the educational resources and support you need to feel your best. Personal, family and financial issues that affect self-care are also taken into consideration.
We offer a wide range of programs and services, including two adult diabetes programs that are accredited by the American Diabetes Association. Diabetes services are available at both our Van Ness Campus and Mission Bernal Campus locations. We offer individually tailored and group education sessions for prediabetes and type 1 and type 2 diabetes self-management, including glucose monitoring, nutrition guidelines, carbohydrate counting and meal planning, medication management, insulin pumps and exercise classes. Our diabetes support groups give you the chance to share and learn from others with diabetes, including those using insulin pumps.
We also provide special services for diabetes in pregnancy, ensuring mom and baby stay healthy throughout pregnancy and after birth.
CPMC partners with Stanford Children’s Health Endocrinology and Diabetes Programs for pediatric diabetes care.
