We help you stay physically active and independent in your community, and we offer additional support coordinating with caregivers if you have in-home care or choose to live in a continuing-care facility.
We also make it a priority to support your family members and friends who help care for you by providing easy access to health information and support services. For example, PAMF’s Successful Aging Shared Medical Appointment series, for those age 65 and older, brings together people on the aging journey to help develop preventive and proactive approaches to aging successfully. Through our internet-based service, My Health Online, family members can email doctors, check test results and participate in your care. We also collaborate with some retirement communities, nursing homes and continuing care facilities.
